BMI and Weight Loss Treatment: What You Need to Know About Eligibility

BMI is the primary eligibility threshold for GLP-1 weight loss treatment — but it is not the whole picture. Here is how clinicians use BMI, its limitations, and what else matters.

What BMI threshold qualifies for GLP-1 treatment?

UK clinical guidelines recommend GLP-1 weight loss medication for adults with a BMI of 30 or above. For individuals from South Asian, Chinese, Black African, African-Caribbean, or Middle Eastern backgrounds, the threshold is lower — typically 27.5 — because metabolic risk (including type 2 diabetes and cardiovascular disease) occurs at lower BMI values in these populations.

BMI is calculated by dividing weight in kilograms by height in metres squared. It is a population-level screening tool, not a precise measure of individual health risk. A BMI of 30 in a 5ft 4in woman carries different clinical implications than the same BMI in a 6ft 2in man.

Why BMI alone is not enough

BMI does not distinguish between fat mass and lean mass. A highly muscular individual may have a BMI above 30 with minimal metabolic risk. Conversely, someone with a BMI of 27 but significant visceral fat (fat around the organs) may have a higher cardiovascular risk than their BMI suggests.

What weight-related conditions improve eligibility?

Patients with a BMI between 27 and 30 who also have weight-related health conditions may still be eligible for treatment. Conditions that strengthen the clinical case include type 2 diabetes or pre-diabetes, high blood pressure, high cholesterol, obstructive sleep apnoea, polycystic ovary syndrome (PCOS), and non-alcoholic fatty liver disease (now termed MASLD).

The presence of these conditions means that weight loss is not merely cosmetic — it is medically indicated and likely to produce measurable health improvements beyond the number on the scale.

What conditions make treatment unsuitable?

Certain medical conditions are absolute or relative contraindications to GLP-1 treatment. These include a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2 (MEN2), a history of pancreatitis, type 1 diabetes, severe kidney disease (eGFR below 30), and active eating disorders.

Pregnancy and breastfeeding are also contraindications. If you are planning a pregnancy, Wegovy should be stopped at least 2 months before attempting to conceive; Mounjaro at least 1 month before. Your EVERLean doctor will review all of this during your assessment.
